Example 1: 30 Internal Medicine programs [$99.00 + (10 x $19.00) + (10 x $23.00)] = $519
Example 2: 23 Internal Medicine programs [$99.00 + (10 X $19.00) + (3 X $23.00)] + 7 Family Medicine programs [$99.00] = $457
